THE PARTICIPANT IS A MEMBER OF THE ARC OF MID-MICHIGAN.*

Fees will be due monthly and should be paid according to the payment schedule, to avoid a $15 late fee. (Monthly payment schedule will be sent once form is submitted)*

If for any reason The Arc of Mid-Michigan cancels connections, which you have already paid for in advance, a credit will be applied to your account.*

The Arc of Mid-Michigan allows a credit to be given for 3 missed days, per program year, that have been paid for in advance.*

After the 3 days have been used, no refund or credit will be given for days that the participant does not attend.*

All credit card transactions for program fees can be processed through The Arc of Mid-Michigan either by a secure Square invoice sent via email, or over the phone by calling our office at (989) 723-7377.*

A processing fee of 3.5% is applied to all credit card transactions.*

I agree to pay the amount indicated on the 2025-2026 monthly payment schedule.*

I understand that The Arc of Mid-Michigan is transitioning to electronic communication to help preserve resources and operate more efficiently. I agree to receive program information, updates, and invoices via email and text. A phone number that can receive text messages and a valid email address are strongly encouraged to ensure I stay informed.*
